Notice of Privacy Practices (HIPAA) – ChuukLink Interpretations and Translations

This Notice of Privacy Practices describes how ChuukLink Interpretations and Translations may use and disclose Protected Health Information (PHI) and your rights under the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

 

1. Our Responsibilities

  • We are required by law to maintain the privacy and security of PHI.

  • We will promptly notify you if a breach occurs that may compromise your PHI.

  • We must follow the duties and privacy practices described in this Notice.

 

2. How We May Use and Disclose PHI

We may use and disclose PHI only as permitted by HIPAA and Business Associate Agreements (BAAs) with Covered Entities, including:

  • Treatment: Assisting healthcare providers with accurate communication during medical encounters.

  • Payment: Supporting billing and claims related to interpreting services.

  • Healthcare Operations: Quality assurance, compliance audits, and training purposes.

  • As Required by Law: Responding to court orders, subpoenas, or government oversight.

We will not use or disclose PHI for marketing, sale of information, or non-authorized purposes without written consent.

 

3. Your Rights Regarding PHI

You have the right to:

  • Access your PHI (request copies).

  • Request amendments if information is incorrect.

  • Restrict disclosures in certain cases.

  • Receive an accounting of certain disclosures.

  • Receive a paper or electronic copy of this Notice.

 

4. Safeguards

We implement:

  • Encryption for PHI in transit and at rest.

  • Secure video platforms (Zoom for Healthcare, Doxy.me, VSee, etc.) under BAAs.

  • Access controls, audit logs, and staff training to ensure compliance.

 

5. Breach Notification

If PHI is compromised, we will:

  • Notify the Covered Entity (and you, if applicable) without unreasonable delay.

  • Provide details about the incident, what was affected, and corrective actions taken.

 

6. Changes to This Notice

We reserve the right to revise this Notice and make the new Notice effective for all PHI we maintain. Updates will be posted on our website at chuuklink.com.
